Your initial consultation appointment with us is necessary to thoroughly diagnose and evaluate your condition and discuss treatment options. The doctor utilizes a variety of diagnostic tools and techniques to determine how to proceed with your treatment. Following this initial consultation, we will schedule your treatment. For those patients who are in pain, we may initiate treatment at the first appointment. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

 

 

 

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

NOTE: A parent or guardian must accompany all patients under 18 at the consultation visit.

 

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves, history of infective endocarditis, congenital heart disease, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.) If possible, bring a list of your medications with you to the consultation. 

 

If you have taken premedication (antibiotics) prior to dental visits for your joint replacement surgery in the past please consult with your orthopedic surgeon if you still require premedication or have questions about this topic. 

 

If your previous dentist has taken recent x-rays (within 6-months), you may request that they email them to our office. We can also call your dentist to request on your behalf. In most cases, we will need to take additional x-rays at our office.
